Carnarvon Medical Service Aboriginal Corporation is an Aboriginal Community Controlled Health Service established in 1986. The Carnarvon Medical Service Aboriginal Corporation provides primary Aboriginal health care services to the Aboriginal communities of Carnarvon and the surrounding areas and aims to provide the highest standard of patient care whilst incorporating a holistic approach towards diagnosis and management of illness. 
About the Opportunity
The Carnarvon Medical Service Aboriginal Corporation is currently seeking a highly motivated and enthusiastic individual with a pa
ssion for community health to join our team in the role of Home and Community Care (HACC) Coordinator. This is a six month maternity leave cover with the possibility of extension.
As the HACC Coordinator you will be responsible for managing and coordinating the provision of high quality, customer focused Home and Community Care Services to clients of the HACC program in Carnarvon and the Gascoyne Region. You will oversee the provision of a range of services for clients who are frail aged, or people with disabilities and their carer's who need additional support to enable them to live comfortably in their own homes for longer.
This unique role provides an exciting and challenging opportunity to work within a multidisciplinary team providing comprehensive health care services to the Carnarvon community in the North West Gascoyne region of Western Australia. The Carnarvon Medical Service Aboriginal Corporation prides itself on the delivery of primary health care services in a supportive, quality, effective and efficient manner to meet the health care needs of the local Aboriginal and Torres Strait Islander peoples.
The successful candidate will have exceptional communication skills, demonstrated caring and respectful attitude towards all clients, strong organisational skills and the ability to lead and coordinate a small team of support workers.
About Carnarvon
Carnarvon is a coastal town situated approximately 900kms north of Perth, Western Australia. It lies at the mouth of the Gascoyne River where it meets the Indian Ocean. Carnarvon is the gate way to the Ningaloo Coast, with the popular Shark Bay world heritage site to the south of the town and the Ningaloo Reef to the north. Carnarvon is home to approximately 7,000 people, with a large Aboriginal community comprising of 15% of the town's population. The town has a fascinating indigenous and pioneering history and is famous for the abundance of fresh fruit, vegetables and seafood produced in the region.
