About The Role
Anglicare Southern Queensland’s Metro South Community Services team are passionate, values-driven professionals working together to affect real outcomes in our community. We’re currently looking for an HCW Team Leader to join our multi-disciplinary team and contribute to the delivery of exceptional client care across our region.
As our Home Care Worker Team Leader you will work under the direction of the Care Coordinator and closely with our nursing staff and the wider team to assist with the facilitation and monitoring of individualised care plans which meet the needs of our clients. With a strong and growing focus on consumer directed care services, you’ll be responsible for initial and ongoing relationship management with clients, package coordination, rostering, client off load and reporting.
You will also provide day-to-day supervision and leadership of a team of Home Care and Support Services Workers, including assisting with the recruitment and orientation of new staff, providing guidance and support, assessment of work practices, client documentation and other related activities. You will frequently travel within your designated zones to provide hands-on support to your team of Home Care and Support Service Workers, conduct initial home visits with clients and from time to time carry out care-related tasks.
The successful applicant will have:
  • Certificate IV in Home and Community Care or equivalent;
  • Current First Aid Certificate;
  • Current unrestricted Queensland drivers licence;
  • Ability to effectively manage teams;
  • Exceptional interpersonal, written and verbal communication skills and an intermediate level of computer skills;
  • Current National Police Certificate or eligible to receive one prior to commencing employment.
Previous experience in a senior or supervisory role, or experience in facilitating non-clinical community based services is highly desirable.
